*Signify Health is hiring part-time, board-certified nurse practitioner employees *to conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. *Part-time clinicians based in New York State enjoy expanded employment opportunities, *includingthe ability to work a flexible schedule. You may also be eligible for certain benefits, like dental and vision insurance, plus 401K match (match-eligible after one year).
* *Conduct in-home visits that focus on a person's overall health and well-being and identify potential barriers to care*
* *Increase your per-visit rate up to 200% with our Market Multiplier\**
* Earn additional compensation via our pay-per-test model (for visits where the health plan member qualifies to receive additional diagnostic and preventive tests)
* Shifts available Monday - Sunday (no nights/weekends required)

* Pre-scheduled visit in-home w/ health plan members

These visits include a medication and medical history review, as well as a physical evaluation performed in a health plan member's home. Our part-time NP employees can earn a competitive wage (or supplement their current income) while managing other personal and clinical commitments. You will be provided a pre-populated, patient-specific form for each evaluation, and do not prescribe medicine, order lab tests, or alter people's current treatment regimen.

*Part-time Signify Health clinicians:*
* Receive all supplies from the company
* Are eligible for current compensation incentives when meeting program requirements (incentives subject to change)
* Are paid biweekly based on completed visits (payment managed by Signify Health Payroll)
* Can decide the schedule flexibility that works best for the individual clinician's needs
* Offer at least one day of availability every 60 days to maintain part-time employment status
* May be eligible for dental and vision insurance, plus 401K (match-eligible after one year). _However, it is not mandatory to elect these benefits._
* Maintain the flexibility to perform services for other companies during part-time employment with Signify Health
